Advance Health Directives - a Dying2Know Day event

Before creating an Advance Health Directive (AHD) it is important to have a good understanding of what advance healthcare planning is, and what is involved in the planning process. AHDs are your choices for your future health and personal care goals.

The creation of an AHD is the best way to make your preferences known regarding the type of treatment you would prefer if you were unable to communicate or make decisions for yourself in a time that is often difficult for family members. An AHD is a way to respect your values, beliefs and preferences.

This informative conversation time will focus on your AHD, question time, and the option of completing your personalised AHD and having it signed by an authorised witness on the evening.
